Community Center for Integrated Health

The Community Center for Integrated Health (CCIH) provides integrated services through expanded access to primary care, mental health and substance abuse services.

This program serves those presenting with mental health concerns or co-occurring mental illness and SUD (COD).

Collaborative Organizations:

• The Child and Family Guidance Center provides counseling services for children, adolescents and families.
• Optimus Healthcare provides primary healthcare services, HIV and Hepatitis testing, blood pressure screening, and wellness support.
• Greater Bridgeport Community Mental Health Center provides access to Peer Recovery Support services, crisis intervention and inpatient treatment.

CCIH is creating a “No Wrong Door” service delivery system by creating an umbrella to cover the needs of the most fragile populations in the Bridgeport area. CCIH will increase and expedite access to primary care, mental health and substance abuse treatment. Providing vital support and resources to increase overall quality of life.
