A CARF-accredited, short-term, intensive 3.7R level of care inpatient program for adult men struggling with co-occurring substance use and mental health. Horizons offers a range of treatment based on professional best practices and evidence-based treatments, such as motivational interviewing, cognitive-behavioral therapy, seeking safety, and medication-assisted services for opiate dependence.

The goal of our program is to prepare individuals for long-term stable recovery in the community, and an important component of this is to carefully develop an individualized aftercare and discharge plan. This may include referrals to a variety of housing, outpatient addiction treatment, and mental health services.
Clients who complete the program are encouraged to become members of our alumni group and participate in offering hope to those who are beginning their own recovery process.
